Is medical weight loss covered by health insurance?

Medisch gewichtsverlies is in Nederland partially reimbursed by health insurance. The basic insurance covers obesity counselling under certain conditions, but reimbursement of weight-loss medication is still not standardly arranged in 2026. Which reimbursement applies depends heavily on the diagnosis, the form of treatment, and the individual insurer. This article answers the most frequently asked questions about the reimbursement of weight loss through health insurance.

What conditions apply to the reimbursement for weight loss?

To be eligible for reimbursement for a weight loss programme through the basic health insurance, there must be a medically diagnosed weight problem. This generally means a BMI of 30 or higher, or a BMI of 25 or higher in combination with a weight-related condition such as type 2 diabetes or high blood pressure. Guidance must be provided by recognised healthcare providers.

The general practitioner plays a central role in this process. A referral is necessary in most cases before an insurer will reimburse treatment. In addition, for most reimbursed programmes, it involves a combined lifestyle intervention, an approach in which nutrition, exercise and behavioural change are addressed together by recognised professionals.

People who do not meet the medical threshold values, or who choose a form of treatment that falls outside the recognised guidelines, are generally not eligible for reimbursement from the basic health insurance.

What does the basic health insurance policy cover regarding being overweight?

The basic insurance covers in 2026 the combined lifestyle intervention (GLI) for people with obesity or severe overweight with complications. This is a structured programme of at least two years, carried out by a recognised healthcare provider, which focuses on sustainable behavioural change in the areas of nutrition, physical activity and mental health.

Conditions for reimbursement from the basic insurance include:

  • A BMI of 30 or higher, or a BMI of 25 or higher with weight-related comorbidity
  • A referral from your GP or specialist
  • Participation in an approved GLI programme
  • The excess applies

Surgical procedures such as a gastric reduction are also reimbursed in specific cases, but this is subject to additional medical criteria and an extensive preliminary pathway. Simple dietary guidance or coaching without a recognised GLI status falls outside the basic reimbursement.

Are GLP-1 medications such as Ozempic covered by health insurance?

GLP-1 medication is in the Netherlands not standardly reimbursed for weight loss via the basic health insurance. In 2026, GLP-1 receptor agonists will only be reimbursed when prescribed for the treatment of type 2 diabetes, not purely as a means for medical weight loss. For people without this diagnosis, reimbursement via the basic health insurance is currently not applicable.

There is much debate in the Netherlands and internationally about the reimbursement of GLP-1 medication for obesity. Health insurers and policy-makers are weighing the medical effectiveness against the high costs and high demand. For the time being, there is no structural reimbursement for the use of this class of medication for overweight without a comorbid condition.

People who want to use GLP-1 medication for weight loss and are not eligible for reimbursement generally pay the costs themselves. A doctor must always assess and approve the treatment before medication is supplied, regardless of whether reimbursement applies.

What if health insurance doesn't cover it?

When health insurance does not cover a weight-loss programme or medication, people can choose a self-funding pathway. This means that the costs will be entirely at your own expense, but access to medical guidance and any necessary medication will remain possible via registered healthcare providers or online medical platforms.

A self-funded treatment pathway has a number of practical advantages. No GP referral is required, the pathway can start more quickly and people have greater freedom of choice regarding the type of treatment. At the same time, it is important that, even with self-funding, a doctor assesses the situation and determines whether a treatment is medically justifiable.

Transparency regarding costs is essential in this regard. People who are considering paying for a weight loss programme themselves would do well to obtain clarity in advance about the total costs of the programme, including any repeat deliveries or follow-up consultations. This prevents surprises afterwards.

Does the supplemental insurance cover a weight loss programme?

Some supplementary insurances offer a partial reimbursement for weight-related care that falls outside the basic insurance. Think of dietary advice, lifestyle coaching or certain forms of support for overweight. Coverage varies greatly per insurer and per package.

What supplementary insurance policies typically do and do not cover:

  • Well: dietary advice from a registered dietitian, lifestyle programmes outside the combined lifestyle intervention (GLI), certain forms of coaching
  • Note: prescription weight loss medication, treatments without medical indication, programmes without a recognised healthcare provider

It is advisable to consult the policy conditions of your own supplemental insurance or to contact the insurer. Reimbursements are determined annually and can change from year to year. Therefore, supplemental insurance may offer a partial solution, but rarely covers a complete medical waste management pathway.
