When are you eligible for weight loss medication?

People are eligible for weight loss medication when their BMI is above a certain threshold, possibly combined with weight-related health problems. A doctor always assesses individually whether treatment is medically justifiable. The questions below provide a clear overview of the criteria involved in this process.

Welke BMI heb je nodig voor afvalmedicatie?

For GLP-1 medication, a BMI of 30 or higher is generally the starting point. For people with a BMI between 27 and 30, treatment can also be considered, but the presence of weight-related conditions such as type 2 diabetes or high blood pressure is then an important additional requirement. A doctor ultimately decides on a case-by-case basis.

The BMI threshold is not an arbitrary limit. A BMI of 30 or higher is considered obesity, at which point the health risks associated with being overweight demonstrably increase. GLP-1 medication has been developed for individuals for whom weight poses a medical risk, not as a tool for mild overweight. That distinction is important because it explains why a doctor always assesses an application individually and does not automatically approve it based on a single number.

Individuals with a BMI just below 30 who are already suffering from weight-related health problems fall into a grey area. In such cases, the doctor considers the overall health situation, not just the BMI score.

Which health problems increase the chances of approval?

Weight-related conditions increase the chance of approval for medication for weight loss, particularly when the BMI is between 27 and 30. Conditions that doctors consider in this regard include type 2 diabetes, high blood pressure, high cholesterol, sleep apnoea, and cardiovascular diseases. With a BMI of 30 or higher, these conditions play a less decisive role, but they do influence the urgency of treatment.

The reasoning behind this is medically sound: when being overweight has already led to measurable health damage, the need for treatment is greater. A doctor then weighs whether the expected health benefits of weight loss outweigh the risks and side effects of medication. The more health problems directly linked to weight, the stronger the medical justification for treatment.

Why aren't you automatically eligible if your BMI is high enough?

A high BMI is a necessary but not sufficient condition for weight loss medication. A doctor also considers other factors, such as pre-existing conditions, medication use, kidney function, cardiovascular problems, and pregnancy. Some of these factors make GLP-1 medication unsuitable or even dangerous, regardless of weight.

For example, there are conditions for which GLP-1 medication is contraindicated, such as certain thyroid conditions or a history of pancreatitis. Certain combinations of medications can also be a reason not to prescribe. A doctor assessing the application will look at the complete medical picture, not just the weight.

This is also why a medical intake is mandatory. Criteria for weight-loss medication cannot simply be ticked off via a BMI calculator. The assessment requires medical knowledge and responsibility, which always lies with a doctor.

How does the medical assessment proceed with an online application?

When applying online for medication for erectile dysfunction, the applicant first completes a detailed medical questionnaire. Based on the answers, a BIG-registered doctor assesses whether treatment is appropriate and medically advisable. This usually takes place within 24 hours on weekdays. Only after approval is a prescription issued and sent to the pharmacy.

The process is entirely digital, but its content is equivalent to a regular medical assessment. The questionnaire asks about weight, height, existing conditions, medication use, and relevant medical history. The doctor uses this information to determine if the criteria for weight loss medication apply and if there are any contraindications.

Following approval by the doctor, the pharmacy independently assesses the prescription before the medication is processed and dispatched. Prescription-only medication is never supplied without this double medical check. More about this process is explained on the page Hoe it wurket.

What if you've tried to lose weight before?

Previous failed attempts to lose weight are not a barrier to an application. They can even be a relevant factor in the medical assessment. When someone demonstrably struggles with weight loss despite lifestyle changes, it can strengthen the medical necessity for support with medication.

GLP-1 medication is not intended as a first step, but as part of a broader process for people who have not achieved sufficient results with weight loss through diet and exercise. The doctor considers how long someone has struggled with being overweight and what steps have previously been taken. This makes the assessment person-centred rather than purely numerical.

It is important to note that medication is not a substitute for lifestyle changes. Results vary from person to person and are strongest when medication is combined with adjustments to eating and exercise habits. A doctor will also take this into account during guidance.

When is waste medication not a suitable option?

Waste medication is not a suitable option in cases of specific medical contraindications, during pregnancy or breastfeeding, with severe kidney or liver problems, or with a history of specific conditions incompatible with GLP-1 medication. Individuals with a BMI below the threshold without weight-related conditions are also generally not eligible.

Furthermore, medication is not suitable as the sole measure for people who are unwilling to also work on their lifestyle. The effect of GLP-1 medication supports weight loss, but does not replace behavioural change. Those who see medication as a quick fix without further adjustments to eating habits or exercise will likely be disappointed with the long-term results.

Finally, not everyone who wishes to be considered will actually receive approval. The doctor decides independently and can reject an application if the medical situation warrants it. This is not a failing of the system, but an essential part of responsible medical care.
